Contact the clinic by calling 360-384-0464 – a staff member will gather the necessary information regarding your concerns.
This information will be forwarded to the nursing team for review and triage.
A nurse will contact you as soon as possible to provide assistance, guidance, and to schedule an appointment if needed.
You can expect to receive a call back within the same day, but no later than 24 hours.
Typically, follow-up appointments are scheduled before you leave the clinic, after visiting your primary care provider (PCP).
If this did not happen, please call the clinic at 360-384-0464 to request an appointment with your PCP. These appointments are usually scheduled in advance, typically 4 to 12 weeks ahead, depending on the reason for the follow-up and the availability of your PCP.
We recommend scheduling yearly wellness exams with your primary care physician (PCP). For younger children, these appointments may be more frequent.
These appointments are scheduled in advance.
Occasionally, you may receive a reminder call from your care team offering an appointment with your PCP.
You can call the clinic at 360-384-0464 to schedule an appointment for a pregnancy test. Your primary care provider (PCP) will be notified of your request, and an appointment will be arranged as soon as possible.
If the test is positive, you will be offered a follow-up appointment for an initial OB consultation. During this appointment, all necessary labs, ultrasounds, and referrals will be ordered.
While we do not provide ongoing prenatal care, we will stay in touch throughout your pregnancy and will reconnect with you and your newborn after your delivery for postpartum and newborn care.
During your pregnancy, you will primarily see your OB provider, but we are also available for any acute care you may need.
Routine lab and imagining results are typically reviewed within 3 to 5 business days. You can expect a call or a MyChart message from the care team with your results and recommendations.
Patients will receive results through MyChart or by phone, even if the results are normal.
The clinic or care team will contact you directly if any results require urgent discussion.
Disability forms, school/work forms, FMLA requests, and similar paperwork may take 5-7 business days to complete.
Your primary care provider (PCP) may request an appointment to complete these forms, which could take longer depending on your PCP’s availability.
These types of forms are typically completed by your PCP and are not expected to be filled out by other providers.
Providers are available by phone during the hours that the clinic is closed by calling 360-715-2447.

Follow the instructions in the link to create a username, password, and access your MyChart account.
MyChart messaging is best for non-urgent questions, such as: clarifying instructions from a recent visit, referral questions, billing questions, or general questions that do not require an in-person visit.
Do not use MyChart for new worsening symptoms, medication reactions, mental health crisis, medical emergencies, or urgent medical concerns.
If you are experiencing any of the above, please call the clinic directly or go to the nearest emergency department or urgent care.
